Medication at School
If a student needs to take prescription or non-prescription medication at school, please review this information. Medication must be in the original labeled container as dispensed or the manufacturer's labeled container.
- The medication label must contain the student's name, name of the medication, directions for use, and date .
- This form must be completed each year. In addition, if any changes are made to medications, this form must be completed again.
Non-Prescription or Prescription Medication Authorization Form
This form must be completed when a student needs prescription medication and/or NON-prescription medication during school hours.
